TEAM DIAGNOSTIC
When persistent engagement, morale or relationship problems resist quick solutions, the visible issue may be only part of the story. A Team Diagnostic helps leaders understand what is happening beneath the surface before deciding how to address it.
Through confidential interviews, leaders and employees share their experiences across 15 aspects of the employee experience. Their individual comments are combined into themes, protecting confidentiality while revealing patterns, strengths and concerns that may otherwise remain hidden.

HOW THE TEAM DIAGNOSTIC WORKS
CONFIDENTIAL INTERVIEWS | Leaders and employees participate in private, one-to-one conversations. The interviews explore what is working well, what may be getting in the team’s way, and how people are experiencing the workplace.
DETAILED REPORT AND RECOMMENDATIONS | The interview findings are consolidated into themes, without attributing comments to individuals. Leaders receive a detailed report with practical recommendations related to team effectiveness, leadership development, employee engagement and other needs revealed through the process.
TEAM CONVERSATION AND ACTION PLANNING | The findings are shared with employees in a facilitated session. Together, the leader and team identify priorities, discuss possible solutions and establish practical commitments for moving forward.
THE FIVE DYSFUNCTIONS OF A TEAM
Based on Patrick Lencioni’s Five Dysfunctions of a Team model, these programs help leadership teams examine five conditions that strongly influence team performance: trust, productive conflict, commitment, accountability and collective results. The work gives teams a practical language for discussing patterns that can otherwise be difficult to name. Programs can be delivered as a focused workshop or extended through coaching circles that help teams apply the concepts to current challenges.
5D WORKSHOP
This one-day workshop helps a leadership team assess its strengths and challenges across the five areas. Participants explore the model, review the team’s high-level assessment results and discuss where perceptions align or differ.
Practical activities help the team identify the behaviours influencing its performance and choose specific ways to strengthen trust, engage in more productive conflict, build commitment, reinforce accountability and focus on shared results.
5D COACHING CIRCLES
These five one-hour facilitated conversations help the team continue applying the model after the initial workshop. Each circle focuses on one of the five areas and connects it to the team’s current experiences, relationships and workplace challenges.
The circles keep the learning alive while giving team members repeated opportunities to practise more candid conversation, shared problem-solving and mutual accountability.
IDEAL TEAM PLAYER WORKSHOP
This workshop explores three qualities that Patrick Lencioni identifies as important to effective teamwork: humility, hunger and people smarts. Participants consider their individual strengths and development opportunities, then examine how these qualities affect the way they contribute to the team.
The conversation can help teams become more candid about individual behaviour without losing sight of collective responsibility. It is particularly useful when a team understands the Five Dysfunctions model but needs to examine more closely how each person contributes to its progress.
CUSTOMIZED TEAM PERFORMANCE WORKSHOPS
Every team has its own history, pressures and patterns. A standard program is not always the right answer.
Customized Team Performance Workshops are designed around the team’s specific needs. Sessions combine practical tools, facilitated conversation and activities that help participants examine how they work together and identify meaningful changes they can make.
COMMUNICATION & TEAM EFFECTIVENESS | This workshop helps participants recognize how communication patterns, assumptions and non-verbal signals affect trust and collaboration. Through practical activities and facilitated discussion, team members build greater awareness of how they affect one another and identify ways to communicate more effectively.
EMPLOYEE SURVEY DEBRIEF & ACTION PLANNING | Survey results show where concerns may exist, but they do not always explain what is driving them. This facilitated session helps employees interpret the findings together, identify the issues with the greatest impact and determine what the team and its leader can do differently.
TEAM OPERATING AGREEMENT | A Team Operating Agreement helps team members establish clear expectations for how they will work together. It can be especially useful for a newly formed team, a team experiencing recurring friction, or two groups that are being brought together. Through facilitated conversation, participants agree on practical standards for communication, decision-making, feedback, accountability, escalation and other behaviours that affect their work. The result is a shared agreement the team can use to address problems before old habits take over.
